Intake session

If possible, please download and print out the intake and symptom inventory forms (in the assessment and treatment consent packet below) and bring them with you to the first appointment; however if this is not convenient, please plan to come 15 minutes early to your first appointment so the paperwork may be completed before the intake appointment.
